Abstract
Disparities in healthcare accessibility across populations of rural residents, those living in poverty, the elderly, women, and minorities have been well documented. Problems such as disparities in burden of disease and inequities in access to care are wicked complex problems with spatial components that influence and are influenced by social determinants of health and are particularly expressed in the distribution of poor outcomes from the opioid epidemic. This complexity requires a multidimensional research approach in order to better understand the various domains and their respective levels of influence on at-risk populations. This approach facilitates the planning and provision of health services for opioid use disorder (OUD). This research consists of a multi-step analysis to identify determinants of health that act as indicator variables associated with the spatial patterns of opioid use, addiction, and mortality. To capture any influence of location on OUD outcomes, ordinary least squares, geographically weighted regression (GWR), and multi-scale GWR (MGWR) are employed to develop a risk index that describes demand for OUD treatment across space and population groups. Data are assessed at the ZIP code level (n = 641) for Alabama. OUD outcome variables from the Alabama Department of Mental Health and Alabama Department of Public Health from 2015-2021 are evaluated against potential risk indicator variables; outcomes are stratified by rurality for policy relevance. The OUD risk framework presented here offers an actionable data-driven approach to support resource allocation in OUD intervention efforts for underserved populations including socioeconomically deprived populations and underserved rural communities
